Date of Admission:

The acceptance of a Resident into our Nursing Home involves a special relationship of intimate care. You are assured that we do our utmost to nurse and care for our Residents in all circumstances. However, to do so we have to maintain an efficient establishment at substantial financial cost and therefore we have to define the relationship in business terms. The following are our basic Conditions of Admission for our mutual benefit.

RESIDENCY TERMS

Residency of a room does not constitute a tenancy within the meaning of The Rent Act. We reserve the right to terminate any residency upon giving seven days notice. Such a right will only be exercised if Management considers this to be in the best interests of the Resident and Green Gables Nursing Home.

DRUGS

All drugs should be handed in on admission and visitors are asked not to bring medicines or food without consulting Matron.

CLOTHING

All items of personal clothing must be discreetly marked with the ResidentÕs name at the time of admission. This is to assist the identification of each ResidentÕs clothing when laundered.

VALUABLES

Residents are advised to ensure that jewellery or any other items of value brought into the Home are insured by them.

SPECIAL NURSING NEEDS

In the event of a Resident requiring a full time Specialist Nurse, the cost will be borne by the Resident or person representing the Resident.

ACTIVITIES

To enhance our Residents' quality of life, there is a comprehensive activities calendar arranged and coordinated by our Activities Organiser. This assists our Residents to pursue their hobbies and interests. Special therapies are also included in the fees e.g. reality orientation therapy and reminiscence therapy. Details of all activities can be seen on our notice boards. In-house activities are included in the weekly fees, however we may ask for donations for minibus hire for summer outings.

ENDING RESIDENCY

It is agreed that one monthÕs notice, in writing, must be given by the Resident, or the person representing the Resident, to end the residency at Green Gables. Such notice should commence on the first day of any given month.

DEATH OF A RESIDENT

In the event of the death of a Resident, four weeks fees are charged from that date.

FEES

The Fee at the time of admission is         £                      from the Council

                                                                        £                      from Client Contribution

£                      from the Primary Care Trust

                                                            =          £                      Total Weekly Fee

Fees are payable monthly in advance from the first day of any given month up to and including the last day of that month. They are subject to review without notice. Fees cover 24-hour nursing care, heating, lighting, water, laundry service (not dry cleaning), food and the use of furniture and equipment within the Home.

ADDITIONAL COSTS

The cost of personal requirements such as telephone calls, newspapers, toiletries, chiropody, hairdressing, physiotherapy and dry cleaning will be borne by the Resident or person representing the Resident.

SHORT STAY

For short stay, fees are charged on a daily or weekly basis by prior arrangement.
